New Patient Downloads
We are currently not accepting new patients. All submitted new patient packets received as of Friday, May 8, 2026 will be reviewed by our clinical team. Once your new patient packet has been approved by our provider, staff will enter your information and you will be contacted via text with an appointment date and time. Please understand that this process can take up to 2-4 weeks due to the demand and focus on providing the highest level of care to our patients. We do not accept Ambetter and Medicaid insurance.
